Appointments:
A 24-hour cancellation notice is required or a $40 fee will apply for late cancellation or no-show.
Please submit any necessary documentation or information prior to your initial appointment, including PIP claims and/or a doctor’s prescription.
Draping: You will be appropriately covered at all times during your session.
Consultation: You will have a consultation with your therapist at the start of each session.
Session Termination: You may end the session at any time for any reason.
Behavior: Inappropriate behavior will not be tolerated and may result in legal action.
Right to Refuse Service: We reserve the right to refuse service.
Client Agreement:
I understand that therapeutic massage therapy does not diagnose or heal illness, disease, or any physical or mental disorder.
I acknowledge that massage therapy is not a substitute for medical examination or diagnosis, and it is recommended that a physician be seen for that service.
I understand that this treatment is intended to address and prevent myofascial pain and dysfunction.
I will inform my massage therapist immediately if I experience any pain or discomfort during the session.
I have disclosed all relevant medical conditions and will update the massage therapist of any changes in my health status.
