Cancellation / No Show

A 24-hour cancellation notice is required, or a $40 fee will apply for late cancellation or no-show.

FSA/HSA

If you plan to use your FSA/HSA card, we do require a referal from your doctor with a diagnostic code. Please email the prescription to our office before your first appointment. We can provide you a superbill for your insurance claim.
